Les opinions émises sur le rôle du ligament rond sont réductibles à deux principales:

1. Le ligament rond est un porte-vaisseaux à la tête du fémur Paletta (1820), Cruveilhier (1834), Sappey (1844), Henle, Richet..., etc);

2. Le ligament rond est un agent mécanique (Gerdy, Tillaux, Morel el Duval, John Struthers, Morris, Savory, Humphry, etc.).

Sans discuter ici les arguments favorables ou défavorables à chacune de ces deux propositions, sans nous préoccuper de la signification morphologie du ligament rond, nous désirons seulement communiquer les résultats que nous a fournis l’expérimentation cadavérique.

Une première série d’expériences a été consacrée à l’étude de la force et de la résistance du ligament rond. Malgré la multiplicité de nos recherches, nous ne l’avons jamais trouvé absent; d une manière générale, son volume et sa forme sont en rapport direct avec le volume du fémur. Chez les sujets à complexion peu vigoureuse, le ligament est petit; il est également atrophié chez ceux qui, avant la mort, ont séjourné longtemps au lit sans marcher. Mais, malgré celle atrophie, le ligament conserve une résistance considérable, qu’il est facile de mesurer exactement.

La capsule articulaire incisée et réséquée, on enlève par un trait de scie horizontal la moitié supérieure de la cavité cotyloïde et de l’os iliaque. Le fémur étant fixé dans un étau, il suffit d’appliquer contre la tête fémorale la moitié inférieure du colle, que relient le ligament rond, pour que celui-ci soit sensiblement dans sa situation normale. Pendant qu’un aide maintient, sans effort, les deux os au contact, des poids sont suspendus à la demi-circonférence inférieure du trou obturateur jusqu'à rupture do ligament. Ges expériences, commencées à l’École pratique de Paris avec. Poirier, et continuées ensuite à Montpellier avec M. Mouret, protecteur, ont été faites sur une centaine de sujets. Voici les moyennes obtenues:

Chez le nouveau-né, leligamenl rondsupporte de 7 à 8 kilogr. Chez l’adulte, le ligué ni rond supporte de 30 à 45 kilogr.

Si on fixe chacun des fémurs à un étau et que l’on cherche à rompre les deux ligaments à la fois en agissant sur les deux trous obturateurs, on est obligé, pour arriver à la rupture du ligament, de suspendre des poids dont la somme varie de 60 à 70 kilogr.

Ces simples constatations conduisent déjà à penser que cet organe est un ligament puissant qui peut jouer un rôle dans la mécanique de l’articulation coxo-fémorale.

Les expérimentateurs sont arrivés à des résultats opposés: ainsi, pour ne citer qu’un exemple, tandis que Savory affirme que, dans’la station debout, le poids du tronc repose en partie sur le ligament rond, Humphry soutient que le ligament rond n’est pas et ne peut pas être tendu dans la station debout. Ges divergences doivent provenir de ce que certaines expériences sont trompeuses. Si l’on prend une articulation coxo-fémorale à capsule excisée et dans laquelle les deux os ne sont plus réunis que par le ligamentrond, quand on engage la tête du fémur dans la cavité cotyloïde, le ligament est toujours tendu, ce qui ne répond pas, comme nous le verrons, à la réalité physiologique. Si on expérimente sur une seule articulation, détachée du reste du bassin, il se produit des inclinaisons de l’os iliaque qui peuvent passer inaperçues et fausser les résultats. Il faut donc, pour bien juger, se rapprocher le plus possible des conditions de la statique normale. Voici comment nous avons procédé.

Un bassin entier, avec les deux membres inférieurs appendus, est séparé du tronc; on enlève les parties molles pour que les différents segments osseux soient seulement maintenus unis par leurs appareils articulaires. La capsule de l’articulation cocofémorale reste intacte; par l’intérieur du bassin, les deux cavités cotyloides sont trépanées largement pour que l’exploration de la cavité articulaire soit facile; sous lo ligament rond, est passée eu anse une ficelle dont les deux bouts ressortent par l’orifice de préparation. La pièce étant ainsi disposée, on la fait reposer sur le sol par les pieds; il suffit qu’un aide empêche les genoux de fléchir et maintienne légèrement le bassin dans l’extension pour qu’elle se soutienne dans la station verticale. Le poids du tronc est représenté par un poids de 20 à 25 kilogr. suspendu au bassin soit par le sacrum, soit par la demi-circonférence inférieure des trous obturateurs. Il n’y a plus dés lors qu’à provoquer des changeai nts de position et à noter chaque fois l’état du ligament rond, en l’explorant avec la ficelle, avec des pinces et avec les doigts, car il est nécessaire de combiner ces différents modes d’examen pour éviter les erreurs.

En expérimentant dans ces conditions, nous avons obtenu les résultats constants que nous présentons:.

Le ligament rond est très lâche et ramassé sur luimême dans:

Extension directe du bassin (station debout).

Flexion directe.

Extension et abdication du fémur (maximum de relâchement).

Flexion et abdication.

 

Le ligament rond est lâche mais ramené à sa longueur normale dans:

Extension et addiction.

Extension et rotation en dedans.

Extension et rotation eu dehors.

Flexion et rotation en dedans.

 

Le ligament est tendu dans:

Flexion et addiction.

Flexion et rotation en dehors

 

Le ligament est très tendu et aplati dans:

Flexion avec inclinaison latérale du bassin.

 

Eu interprétant ces faits nous arrivons aux conclusions suivantes:

1. Le ligament rond limite les mouvements de flexion et addiction, de flexion et de rotation en dehors;

2. Il devient nettement suspense du tronc dans la flexion avec inclinaison latérale du bassin lorsque le poids du tronc porte presque en entier sur un des fémurs. Ainsi, la tension du ligament rond de l’articulation coxo-fémorale droite devient extrême, si, le membre inférieur gauche étant fléchi et rejeté en arriéra, le bassin se fléchit en avant, tout en s’inclinant latéralement du côté gauche;

3. Le ligament est lâche dans la station verticale; il n’est donc pas suspenseur du tronc à ce moment. Il n’intervient pas plus activement dans les autres mouvements.

An excerpt from the article by Morris H. Dislocations of the Thigh: their mode of occurrence as indicated by experiments, and the Anatomy of the Hip-joint (1877, pp. 161-170).
