A. Die pathologisch - anatomisehen Merkmale.

Das Milssverhaltnis zwischen der Grosse des Femurkopfes und des Acetabulum ist einer der auffallendsten anatomischen Befunde, der immer als Charakteristikum der congenitalen Luxation galt. Es ist dabei sehr oft der Femurkopf, obgleich er ftir die Pfanne zu gross ist, doch kleiner als ein entsprechendes normales Caput femoris; ja selbst das ganze Femur kann hypoplastisch sein. Die Pfanne dagegen hat, wenn sie auch klein ist, doeh regelmassig einea so stark verdickten, hyperplastisehen Boden, dass sieh die Chirurgen wnnderten, wie leicht es ist, sie operativ zu vertiefen, ohne den Beckenraum zu eroffnen (HOFFA). Regelmggig ist dagegen der obere Pfannenquadrant wenig entwickelt (hypoplastiseh) und bisweilen ist das ganze Becken asymmetrisch ausgebildet. Es ist der Femurkopf gewShnlich so gedreht, dab er die Pfanne nur mit einem Bruchteil der Flgche bertthrt, die bei typischen Gelenken in Kontakt mit ihr steht. Die Pfanne kann aueh nach der Symphyse zu verlagert nnd dadureh der Kontakt mit dem Kopf noch mehr ersehwert sein. Das ist den Chirurgen namentlich oft bei blntigen ErSffnungen der Gelenkgegend aufgefallen, da sie die Pfanne an dem gewohnten Ort nicht zu finden vermoehten. Sehr haufig fehlt das Ligamentum teres.

Diese, besonders yon P. BADE (1907) teils naeh andern Antoren zusammengestellten, tells selbst erhobenen Befunde konnte ieh fast alle als eharakteristische Merkmale der experimentell bewirkten Gelenkanomalie der Unke naehweisen. Ein geringer Substanzverlust des origin~tren Anlagemateriales bringt bier den Stein ins Rollen, der alle andern Vergnderungen nach sich zieht: Die Pfanne wird ftir den Kopf zu klein, ihr Grund aber ist trotzdem - vielleieht dnrch Verdr~tngung des Bildungsmateriales - verdiekt. Durch Wachstumsdruek beeinflussen sieh Pfanne und Kopf so, dab beide defekt werden (die Pfanne besonders an ihrem dorsalen Rand), und dab der Kopf in eine Zwangslage gerat, in weleher er nnr noch geringen Kontakt mit der Pfanne hat. Das Pfannenband kann fehlen. Es ist selbstverstiindlich, dag der Schultergurtel, der originair verkleinert ist, yon Anfang an asymmetrisch zu demjenigen der andern Seite gestaltet sein muss.
