Description

 

1. Beschreibung einer Endoprothese

Endoprothesen (griech.: endo „innen”; Prothese „künstlicher Ersatz”) sind Implantate, die im Körper eingesetzt werden und ein geschädigtes Gelenk ganz oder teilweise ersetzen. Endoprothesen gibt es für fast alle Gelenke der Extremitäten (Kniegelenk, Schultergelenk, Sprunggelenk, Ellbogengelenk ect.), jedoch ist sicherlich das Hüftgelenk, das am meisten durch eine Endoprothese versorgte Gelenk. Allein in Deutschland setzten Ärzte jährlich über 200.000 Patienten ein künstliches Hüftgelenk ein. Die Tendenz ist mit dem stetig steigendem Alter der Menschen stark steigend.

Wir demonstrieren die neue Erfindung daher an der Hüft-Totalendoprothese (Hüft-TEP). Jedoch ist diese Erfindung auch an anderen Gelenkprothesen einsetzbar.

Eine Hüft Totalendoprothese (H-TEP) besteht aus den folgenden Komponenten:
Die Hüftpfanne, welche in die ursprüngliche Hüftpfanne eingebracht wird. Das Inlay, welches in die Hüftpfanne eingebracht wird. Der Hüftkopf, welches im Schafthalsanteil eingebracht wird und kongruent zum Inlay geformt ist, in dem er für die Beweglichkeit der Prothese sorgt. Der Schaft, welcher im hüftnahen Halsanteil mit dem Hüftkopf verbunden wird und an der peripheren Seite in den Oberschenkelknochen implantiert wird. Die Hüftpfanne wird in die ursprüngliche Pfanne eingebracht, nachdem diese vorher bearbeitet wird. Die Pfanne kann je nach Knochenkonsistenz mittels Schrauben im Becken fixiert oder auch zementiert werden. Danach wird ein Inlay in die Pfanne eingebracht, in dem der kugelartige Kopf rotieren kann. Dieser Kopf kommt im Hals des Schaftes zum liegen. Der Schaftanteil, wird in den vorher bearbeiteten Oberschenkelknochen eingebracht und optional zementiert.

2. Die Entfernung des Ligamentum capitis femoris

Bei der oben angesprochenen Bearbeitung der ursprünglichen Pfanne wird insbesondere das Oberschenkelknochenkopfband (Ligamentum capitis femoris, kurz LCF) entfernt.

Dies ist das stärkste Band im menschlichen Körper mit bis zu 350 Kg Zugkraft, welches dafür verantwortlich ist, dass der Hüftkopf nicht aus der Pfanne luxiert. Die Luxation einer gesunden Hüfte ist daher nahezu unmöglich. Anders als die meisten anderen Gelenke, welche durch Sehnen und Bänder geführt werden, wird die Stabilisierung der Hüfte im wesentlichen durch das LCF gewährleistet. Diese Struktur muss bei der Operation entfernt werden und bis heute gibt es für diese Funktion keinen künstlichen Ersatz, weshalb es zu vielen Luxation nach einer Protheseneinsetzung kommt und genau hier setzt die neue Erfindung an.

3. Die Luxation nach Hüftprothesen

Die Luxation einer Hüft-TEP ist als vollständiger Verlust des Artikulationskontaktes zwischen zwei Kunstgelenkteilen definiert.

Bei der Luxation springt in den meisten Fällen der rundliche Kopf aus dem Inlay heraus und beschädigt das Inlay oft zusätzlich, welches in der Folge meist verrutscht. Es fehlt das LCF, welches das Herausgleiten des Schaftanteils mit der Kopfkomponente aus dem Inlay verhindert.

Die jährliche Luxationsrate primärer Hüftprothesen liegt laut Studien zwischen bei bis zu 10% im ersten Jahr nach Implantation. Nach Revisionsoperationen des künstlichen Hüftgelenkes werden in Abhängigkeit vom Patientenkollektiv, Erfassungszeitraum und Prothesentyp Luxationsraten von bis zu 28% beschrieben.

4. Folgen einer Luxation nach Hüftprothese

Die Luxation einer Hüftprothese ist ein traumatisierendes Ereignis für die Patienten. Die Angst vor einer erneuten Luxation begleitet sie meist ein Leben lang und prägt ihren Alltag. Die Luxation bedarf einer umgehenden Reposition. Wenn bildmorphologisch eine Schädigung der Gelenkkomponenten visualisiert wird, ist die operative Revision unumgänglich. Verbunden mit Revisionseingriffen sind die bekannten Komplikationen einer Operation (Infektionen, Thrombosen, Verletzungen umliegender Strukturen u. v. m.). Desweiteren entstehen erneute Krankenhauskosten und die Nachteile eines weiteren Krankenhausaufenthaltes für den Patienten (berufliche und private Arbeitsunfähigkeit).

5. Die Erfindung: Das Tanab-Ligament

Eine schnurartige oder bandartige Struktur, welche die anatomische Rekonstruktion des LCF darstellt, welches bei der H-TEP Implantation entfernt wird. Diese Struktur nennen wir das Tanab-Ligament (TL). Das TL entspringt aus der Hüftpfanne und durchläuft das Inlay durch eine Öffnung und dringt in den Kopf ein, welches ebenfalls durch eine Öffnung für das TL durchlässig ist. Der Halsanteil der Schaftkomponente ist wiederum durch eine Eintrittslücke für das TL passierbar und leitet dieses durch einen oder mehreren Austrittspunkten aus dem Schafthalsanteil wieder heraus. Nach Ausleiten aus dem Schafthalsanteil kann das TL nun gekürzt und fixiert werden. Dabei kann das TL a priori in die Pfanne integriert sein oder durch ein Konnetierungszwischenstück mit der Pfanne verbunden werden und ist somit austauschbar. Zudem besitzt das TL die Fähigkeit sich an seinem Ursprung in der Gelenkpfanne um 360° zu rotieren im und gegen den Uhrzeigersinn. Somit verhindert das TL mit seiner starken Zugkraft und seiner anatomisch angepassten Führung eine Luxation aller Komponenten des Hüft-TEP zueinander. Das TL kann zudem monofilamentär oder polyfilamentärer Morphologie sein. Bei einem monofilen TL wird dieses aus einem Austrittspunkt im Schafthalsanteil ausgeleitet und dort fixiert. Bei einem polyfilamentärem TL verfügt das TL die Möglichkeit in der Peripherie die vielen Filamente aus mehreren Austrittspunkten im Schafthalsanteil auszuleiten und damit das Bewegungsausmaß der Hüfte so zu modellieren, wie es der Operateur für den Patienten für adäquat erachtet. Das TL kann dabei oberflächenbeschichtet sein und als Medikamententräger fungieren. Es kann sowohl von völlig ausgefüllter Konsistenz sein als auch innen hohl. Ausgeleitet aus den Austrittspunkten im Schafthals können die Filamente oder das einzige Filament gekürzt und fixiert werden.

6. Vorteile des Tanab-Ligaments

Die Hauptfunktion des TL ist die Verhinderung der Luxation des Gelenkkopfes aus der Gelenkpfanne und dem Inlay.

Weiterhin gibt das TL die Möglichkeit Medikamente insbesondere Antibiotika zu integrieren, welche im Falle einer Infektion postoperativ oder zur Prophylaxe bei der Implantation sofort lokal ihre Wirkung entfalten können.

Ein polyfilamentäres TL kann durch Kürzung der Filamente die Bewegungsausmaße so modulieren, wie es den Bedürfnissen des Patienten entspricht. Damit können die Winkel der Abduktion/Adduktion, der Innen- und Außenrotation, der Beugung und der Streckung im Hüftgelenk damit freigegeben oder limitiert werden. Somit wirkt das TL ungünstigen Bewegungen zuvorkommend. Das TL ist ermöglicht es selbst bei insuffizienter Gewebespannung, wie z. B. bei älteren Menschen, eine suffiziente Führung des Gelenkes zu gewährleisten. Zudem werden auch etwaige Komponentenfehlpositionierungen, welche als eine mechanische Ursache von Luxationen angesehen werden, durch die TL-Führung kompensiert.

Claims:

The combination of a joint endoprosthesis and a band-like, cord-like structure. A structure that passes through the components of an endoprosthesis and thus prevents dislocation. This structure arises from the acetabulum. It passes through the inlay through an opening and penetrates into the femoral head, which is also permeable through an opening for the TL. The neck portion of the shaft component is again passable through an entry gap for the TL and thus allows the discharge of the TL through one or more exit ports out. After discharging from the shaft portion, the TL can now be shortened and fixed. The various components of a prosthesis become permeable through one or more openings for ribbon-like structures. The TL can rotate 360 ° clockwise and counterclockwise at its origin in the pan around its own axis. By individual shortening of the TL filaments, the determination of the movement angles by this band-like structure is made possible. The TL can be monofilamentary or polyfilamentary. In the joint socket, the TL can be integrated a priori or connected to the socket by a connecting component and is thus exchangeable. The TL can act as a drug carrier such as antibiotics by surface coating. The TL can be completely filled inside or hollow inside and thus filled. The shank neck may have one or more openings from which the TL or multiple filaments of the TL may be discharged. The TL can simulate the ligaments removed before implantation at various joints, e.g. B. the cruciate ligaments on the knee joint in total knee prostheses.
