Ligament inter-articulaire. Quoique occupant l'intervalle des surfaces, il n'est pas, comme il le semble d'abord, contenu dans l'articulation, à cause de la gaîne synoviale dans laquelle il se trouve, et que la dissection en sépare avec beaucoup de facilité. Il n'est point, comme les autres ligaments inter-articulaires, fibrocartilagineux. C'est un véritable faisceau ligamenteux qui, né de l'enfoncement raboteux qu'offre la tête du fémur, va gagner, non pas le fond inégal de la cavité cotyloïde, comme le disent la plupart des auteurs, mais les deux extrémités de l'échancrure cotyloïdienne, par deux faisceaux qui se séparent en cet endroit pour aller à chacune de ces extrémités, où ils se fixent au dedans du ligament cotyloïdien, en formant avec lui un trou de l'échancrure précédente. C'est le repli synovial, et non le corps fibreux qu'il revêt, qui du fémur va au fond inégal de la cavité cotyloïde ce corps fibreux est tellement disposé par cette insertion interne, que la luxation du fémur dans le trou ovalaire est la seule qui puisse s'opérer sans sa rupture, laquelle est inévitable dans les autres déplacements, surtout dans celui en dehors et en haut. Il est possible qu'alors ce ligament étant rompu, la portion de la gaîne synoviale qui va tapisser le fond inégal de la cavité cotyloïde reste intacte, et serve jusqu'à un certain point à maintenir encore un peu les rapports. Je remarque que, dans la luxation en bas et en dehors, le faisceau attaché à l'extrémité supérieure de l'échancrure peut être rompu seul; que dans celle en haut et en dedans, un phénomène opposé peut avoir lieu en sorte que, dans ces deux luxations, la rupture du ligament inter-articulaire peut n'être et n'est probablement jamais que partielle.

Membrane synoviale. Elle se déploie d'abord dans la cavité cotyloïde, où elle est très-apparente sur le tissu cellulaire rougeâtre qui s'y rencontre, et dont on peut la soulever par l'insufflation à travers l'échancrure qui transmet les vaisseaux. Elle se déploie ensuite sur le ligament cotyloïdien, et descend sur la surface interne de la capsule, à laquelle elle communique son poli, et dont on peut la séparer. Parvenue à la base du col du fémur, elle abandonne le ligament capsulaire, et se réfléchit pour revêtir un tissu fibreux qui forme le périoste de ce col, et qui, formé de fibres parallèles, est beaucoup plus sensible en devant et en dedans que dans les autres sens. De là, la synoviale gagne le cartilage de la tête, qu'elle recouvre partout, excepté à l'insertion du ligament inter-articulaire; là elle envoie un prolongement en forme de canal, qui embrasse ce ligament et l'accompagne jusqu'au tissu rougeâtre de la cavité cotyloïde.
